Health on the Streets – Bathurst & Orange, NSW

Coast & Country Primary Care’s Health on the Streets (HoTS) program is a nurse-led primary care outreach service providing free, low-barrier healthcare for people experiencing homelessness or at risk of homelessness.

From April to June 2026, funded by Western NSW Primary Health Network (WNSW PHN), the Health on the Streets team will be delivering a time-limited outreach service across Bathurst and Orange, working in conjunction with established local services, from trusted community hubs to make it easier for people to access healthcare when they need it.

This is a walk-in service with no appointment required.

Who This Service Is For

This service is for people who:

  • Are experiencing homelessness

  • Are at risk of homelessness

  • Are not currently seeing a GP regularly

  • Need help with their health but are finding it hard to access services

  • Need wound care, health checks, referrals, or help navigating the health system.

    We work closely with local services and can help connect people with ongoing healthcare and support.

❋ What We Can Help With

Our nurses can provide:

  • Health checks and basic medical care

  • Wound care and infection management

  • Blood pressure and blood sugar checks

  • Help with medications

  • Mental health support and referrals

  • Alcohol and other drug support and referrals

  • Sexual health screening and referrals

  • Pathology referrals Vaccination support (where available)

  • Referrals to GPs, community health, and hospitals if needed

  • Telehealth GP appointments where appropriate

  • Follow-up appointments and recall for ongoing care

  • How the Service Works Our nurses visit community hubs in Bathurst and Orange each week. People can walk in during clinic times, speak with a nurse, and receive healthcare and referrals if needed. We aim to:‍ ‍
    ✅ Provide immediate healthcare where possible
    ✅ Follow up people who need ongoing care
    ✅ Connect people into local GP and health services
    ✅ Work alongside local community services and support agencies
