Free Nurse-Led Health Clinics
Health on the Streets – Bathurst & Orange
Coast & Country Primary Care’s Health on the Streets (HoTS) program is a nurse-led primary care outreach service providing free, low-barrier healthcare for people experiencing homelessness or at risk of homelessness.
From April to June 2026, our team will be delivering a time-limited outreach service across Bathurst and Orange, working from trusted community hubs to make it easier for people to access healthcare when they need it.
This is a walk-in service with no appointment required.
❋ Who This Service Is For
This service is for people who:
- Are experiencing homelessness
- Are at risk of homelessness
- Are not currently seeing a GP regularly
- Need help with their health but are finding it hard to access services
- Need wound care, health checks, referrals, or help navigating the health system.
We work closely with local services and can help connect people with ongoing healthcare and support.
❋ What We Can Help With
Our nurses can provide:
- Health checks and basic medical care
- Wound care and infection management
- Blood pressure and blood sugar checks
- Help with medications
- Mental health support and referrals
- Alcohol and other drug support and referrals
- Sexual health screening and referrals
- Pathology referrals Vaccination support (where available)
- Referrals to GPs, community health, and hospitals if needed
- Telehealth GP appointments where appropriate
- Follow-up appointments and recall for ongoing care
How the Service Works Our nurses visit community hubs in Bathurst and Orange each week. People can walk in during clinic times, speak with a nurse, and receive healthcare and referrals if needed. We aim to: Provide immediate healthcare where possible Follow up people who need ongoing care Connect people into local GP and health services Work alongside local community services and support agencies
